We're so happy you will be participating! Please note you only need to fill out this form if you'd like us to make a supply kit for you to complete this project at home. If you already have supplies at home, or if you plan to create your work of art at one of our physically-distanced gathering opportunities at PJTC, you do not need to complete this form.

You can do one rock as a family or each family member can paint a separate rock. With that in mind, how many rocks will you be needing? (Write "0" if you don't need rocks but still need the other art supplies.)

Delivery or Pickup at PJTC?

I will pick up the kit at PJTC on either the morning of August 30 (LBSRS) or September 18 (everyone).Please Deliver to my house (we'll do our best--but please reserve this option for those who really need it!)You will be notified via email with more details about pick up as the opportunity approaches.
