We are thrilled you will be joining us for Social Justice Shabbat Services, sponsored by PJTC Social Justice Group, on Friday, April 22 and Shabbat Services, on Saturday, April 23.  This form can be used for both online and in-person attendance.

If you would like to come to Services, please bring proof of Covid vaccination or a negative Antigen test. 
  
This weekend's services are expected to have over 50 people, which means even children ages 2-11 not eligible/those who have not been vaccinated must show a negative Antigen Test. All safety protocols including masking and social distancing will apply.

You will receive a zoom link in your confirmation email.

Before coming to PJTC in person, we are reaching out to you about COVID-19 novel coronavirus. In order to keep you, your loved ones, and members of the congregation safe, we implemented a brief exposure and symptoms screening which we are asking all congregants to complete before attending any in person activity on the PJTC campus.

We know plans can change when it comes to children. If there is any chance at all that children will be joining you, please click "yes" to read our updated policies pertaining to children at services at PJTC.
Following the most up-to-date guidelines on COVID-19 precautions is important for your health and safety.  Should you develop any new symptoms between answering this questionnaire and the time of your attendance, please seek out medical attention, DO NOT come to PJTC, and quarantine as per CDC guidelines.
